KINDERGARTEN SCREENING
May 18 & 19
Sandusky Central Catholic School Kindergarten Screening will be held at the Holy Angels Church Gathering Space on Tuesday May 18 and Wednesday May 19.

Children must be 5 years old on or before September 30, 2010.
Parent/Guardian must bring the following Required Documents:
(If you are missing any of the required documents, we encourage you to still schedule your appointment at this time. Missing documents can be brought in at a later date.)
Child's Birth Certificate
(Original birth certificate containing raised seal and/or original stamp)
Child's Immunization (Shot) Record
(From doctor or Health Department)
Legal Documents Pertinent to the Child
(Custody, name change, grandparent act, etc., court documents must contain journal entry stamp)
Verification of Parent/Guardian Address
(Utility bill, lease/rental agreement, etc.)
Parent/Guardian Driver's License or Photo ID
